Eating Disorder Plans

We welcome referrals under Eating Disorder Plans. Please provide us with a copy of your EDP template with the following details:

• Nominate the psychological treatment to: Dr Ashlea Broomfield

• Nominate the dietetic services to: Rebecca May

Please include a letter including relevant information which nominates the number of sessions allocated to each individual provider.

People coming to see Dr Ashlea Broomfield under an Eating Disorder Plan also receive a medical assessment and have the option of ongoing medical monitoring as a standard part of their collaborative care.

We provide regular updates to the Primary GP.

Mental Health Care Plans

Ashlea can receive referrals under Mental Health Care Plans. Please provide us with a copy of your Mental Health Care Plan and a letter with relevant information, including the number of sessions allocated.

Team Care Arrangements

We are able to provide collaborative care under Team Care Arrangments. Please provide a copy of the Team Care Arrangement and a referral letter indicating allocated sessions for allied health sessions with Rebecca.

Referrals without Plans

We are happy to receive private referrals.

As Dr Ashlea Broomfield is a Specialist GP, she can also complete the assessment, diagnosis and treatment planning for Mental Health Care Plans and Eating Disorder Plans. To enable collaborative care, we would appreciate a referral specifically requesting this.
